Voluntary Insurance and Coverage of Family Members (CCSS)

The CCSS voluntary insurance is intended for people who do not have a formal income‑generating activity but do have the ability to pay: students, retirees, pensioners, or unemployed persons who receive support from a family member or have informal income. It is different from employer enrollment and the independent‑worker insurance (see the corresponding fact sheets), which apply when there is a formal economic activity.

Documentos requeridos:

Proceso de afiliación: 1. Interview with a CCSS official. 2. Sworn statement about the person’s economic situation. 3. The CCSS determines the monthly fee to pay based on that interview. 4. The applicant (and any dependents they add) makes the first payment. 5. Go to the EBAIS serving your home address to obtain the social security card.

Nota: It is important to verify the specific requirements for adding dependents (spouse, children, or other family members who are supported) directly with the CCSS or in person at a branch, as the information available online is limited.

Nota importante sobre la base mínima contributiva (BMC): Según el Decreto N°44756-MTSS publicado en La Gaceta N°229 del 05 de diciembre 2025, se ha actualizado el ingreso mínimo de referencia o base mínima contributiva (BMC) para el cálculo de las cuotas al Seguro de Salud (SEM) y el Seguro de Pensiones (IVM).

Esta disposición aplicará a partir de la facturación de enero del 2026.
